A fractional growth leader shouldn't spend the first 90 days producing a longer marketing list.

The value of adding senior commercial leadership is better decisions about what the company should do, what it should stop doing and which constraints are actually limiting growth.

For an early or growth-stage MedTech company, those constraints rarely sit inside one function. Positioning affects fundraising. Clinical evidence affects sales enablement. KOL strategy affects adoption. The website affects investor confidence and buyer trust. A sales problem may begin as an awareness, targeting, proof or workflow problem.

The first 90 days should make that system visible.

Days 1 to 30: Find The Actual Constraint

The first month is diagnostic. A senior advisor should review the market, product, evidence, customer, competitive alternatives, current pipeline, sales motion, messaging and internal decision process.

The goal is to determine where growth is getting stuck and why.

That means testing the assumptions underneath the plan. Is the ICP specific enough to guide action? Does the buying committee match how the market actually buys? Is the value proposition built around an urgent problem or the company's favorite feature? Does the evidence support the claim being made? Is sales asking marketing to solve a problem that begins somewhere else?

A company does not need another generic audit that describes everything it already knows. It needs a point of view about the constraint that matters most.

Days 31 to 60: Align the Commercial Story And Motion

Once the constraint is clear, the company needs one commercial premise connecting positioning, audience, proof and action.

This is where teams often discover that the website says one thing, the deck says another and the sales team tells a third version based on who is in the room.

The work during this phase should create alignment around the priority market, buying committee, value narrative, evidence hierarchy, channel strategy and sales motion. It should also clarify what the company is not pursuing right now.

Focus is how a company with limited runway gives a strong idea enough force to move.

Days 61 to 90: Build The Operating Rhythm

Strategy becomes useful when it changes what happens each week.

By the third month, the company should have a practical operating rhythm: priorities, owners, measures, decision points and a sequenced execution roadmap. Marketing, sales, clinical and leadership should know how their work connects to the same commercial outcomes.

This doesn't mean every campaign, asset or launch task has been completed. It means the team can distinguish motion from progress and knows which work deserves resources next.

What Should Be Different By Day 90

By the end of the first 90 days:

  • The company can explain its market, buyer and value clearly and consistently.

  • Leadership agrees on the primary growth constraint and the decisions required to remove it.

  • The sales and marketing motion reflects the real buying process.

  • Evidence, content and KOL activity answer commercial questions the market needs resolved.

  • The team has a prioritized roadmap with owners, measures and a clear sequence.

If a fractional leader leaves the company busier but not clearer, the company just bought another pair of hands.

Senior leadership should change the quality and speed of the decisions the business makes.
